Blunt Nose and Protruding Mouth Improvement Surgery – Noselab Dr. Chayoung Kang

Hello!

This is Dr. Chayoung Kang, the Director of Noselab Clinic.

Today we will talk about a case of rhinoplasty that improved the appearance of a protruding mouth.

Preoperative Design Consultation

< Frontal view >

  • Overall, the nose looked bulbous and blunt.
  • The nasal base was wide which emphasized the bulbous appearance of the nose.
  • The nasal bone and nostrils were asymmetric, which made the nose appear crooked.

  • The space betwen the nasal bridge and glabella was flat and plain.
  • The nose tip was droopy, and the space between the columella and alar was barely visible, to the point that the columella appear retracted.
  • Excessively protruding maxilla.

  • Since the nasal base was too wide, the alar on both sides had different sizes.
  • The shape of the nostrils was asymmetric.

Surgery plan

  1. To correct the bulbous nose, part of the subcutaneous tissue would be removed in order to reduce the skin thickness. The wide shape of the nose would be improved by relocating the alar cartilage, and by using the self-rib cartilage, we can heighten the nose tip while giving it a three-dimensional effect.
  2. To reduce the width of the nose, we would perform a nasal base reduction surgery.
  3. To correct the crooked nose, we would perform a nasal bone osteotomy.
  4. In the preoperative CT scan findings, we estimate that after getting double jaw surgery, there was a separation of the base of the septum and the upper jaw bone, so we planned a septum reconstruction surgery.
  5. To observe improvement in the appearance of the protruding mouth, we planned to perform a nasolabial angle correction.

In the 90-degree side profile view, we can observe that the plain line has become more refined. The droopy nose tip has become sharper, and through the nasolabial angle correction, the protruding maxilla appearance has visibly improved.

From the front, we can observe that the bent appearance has improved significantly, and the wide nasal base has been reduced considerably. Especially, the appearance of the nasal alar asymmetry has improved a lot, and the ACR (alar columella relationship) has become visibily more natural-looking.

When observing the nostril view photos, the right nostril that was bigger, and the nasal base on the right side that was also wider have definitely become more similar.
