Rhinoplasty Korea + upturned nose + asymmetric nostrils

1. Preoperative counseling

The patient visited my clinic to correct the asymmetric feeling and curved nose, showing a lot of nostrils in the front. The skin at the tip of the nose was thick and required correction of nasolabial angle 90 degrees.

The patient had congestion on the left side of the nose, and CT and nasal endoscopy taken at the time of the visit revealed that he was diagnosed with septal deviation and hypertrophic rhinitis.

2. A surgical plan

The lateral osteotomy was planned for the above bent nose correction, and the use of septal cartilage and donated rib cartilage is actively considered for projecting the nose tip correction to sufficiently lower the columella. In addition, the surgery related to the blockage of the left nose had a history of undergoing rhinitis surgery 25 years ago, and septal deviation and hypertrophic rhinitis were diagnosed by CT taken at our clinic, so septoplasty and submucosal resection of the inferoir turbinate using a high-frequency laser were planned.

3. Surgical findings

When he had rhinitis surgery 25 years ago, he had septoplasty at the same time, and his septal cartilage was not in good condition. In the previous hospital, the second half of the septal cartilage was resected too much, and the remaining septal cartilage was severely cross-hatched incision, which required septal reconstruction surgery in a state where the stability of the septal cartilage was lost. Because of this, the left nose became clogged and the tip of the nose was bent, so septal reconstruction surgery was performed.

5. A general review

The patient’s septal cartilage condition above is not very good, so it is a surgery that requires septal reconstruction. Although septal reconstruction surgery is strongly suggested to use self-costal cartilage as much as possible, the patient wanted to use donated rib cartilage, so the entire septal cartilage was reconstructed using donated rib cartilage. The amount of donated rib cartilage used is about three times higher than that of simple cosmetic surgery patients. I made good results after the operation, but He should take good care of it for more than six months after the operation.
